This pressure is keenly felt in Continuing Healthcare, and the move to system working and creating a collaborative and place-based CHC service is an additional tension point.
The balance of pressures is a careful juggling act. The number one priority is always to meet individual needs, but additional conditions continue to be required by the government, authoritative bodies, senior management, workforce, and more. As such, our experts at Liaison Care have been pooling their experience to create a specific package which aims to place the pressures into a box and wrap up a solution, whilst always keeping a person’s needs and clinical quality front and centre.

At a time when financial pressure around CHC spend is at an all-time high; when every place seems to have extraordinary challenges of recruitment and retention; when the appetite to ‘invest to save’ in the CHC space appears to be on the decline, Liaison Care has come up with a clear solution to help manage these issues.
Our risk/gain share model is a direct response to many of the pressures faced by both CHC managers and their teams, and offers a multi-year plan to blend consultancy and a managed service for CHC.
We provide an experienced and clinically-led team with you at our cost, initially providing a managed service for completion of all 3- and 12-month reviews, including backlog cases. Our team will independently quality assess every case file to ensure that each individual’s healthcare needs are aligned, and that they are receiving the correct funding. In addition, our team will complete the eligibility process (Decision Support Tool) for those who have a significant change in needs, ensuring that funding streams are accurate for each individual and minimising the impact of the programme on existing team workloads.
Person-centred needs are at the centre of our work, and package costs to meet identified needs can rise or fall, but we have found that our reviews may also reduce costs over time. This service has a gain share arrangement, so that any savings made is split, allowing potential room for more investment into the service in the first place, as well as cost management for CHC across the system.

Individuals benefit from independent CHC reviews conducted by a team with both expertise and experience. Our assessors have health and social care backgrounds, as well as a passion for ensuring that the individual receives the care they need and they and their families get the support and communication required to assist them on their health journeys.
For local CHC teams, this has the unique advantage of the assurance that constant supervision is not required, as is often the case with temporary staff, unburdening them from the pressure of a growing workload and enabling them to focus on other essential work needs.
