This backlog not only increased pressure on their internal Continuing Healthcare (CHC) team but also led to missed review deadlines. With low retention levels within the team and expensive agency support, the ICB recognised the urgent need for a collaborative partnership to address these issues.
The team immediately began covering a minimum of 15 DSTs weekly, with each one taking approximately 10.5 hours of practitioner time – a resource which would not have been possible without Liaison Care’s team being ready to provide support at the highest standard.
Liaison Care is committed to providing a person-centred approach, executing the excellent standard of care that the business has built a reputation on. The arrangement between Liaison Care and the ICB was made on a cost per case contract, meaning that not only did the ICB receive reassurance that the partnership was making a difference, but that it saw value for those cases completed and freed from the backlog, also ensuring that the individuals involved, and their loved ones received the support and communication they also required.
Liaison Care’s team, through their expertise and commitment, addressed the CHC backlog promptly, covering a substantial number of DSTs each week. 111 cases reviewed for the ICB in total.
The efficient use of practitioner time, approximately 10.5 hours per DST, 1,166 hours of resource in total, allowed the ICB to manage a high volume of cases without overburdening their internal team.
The cost per case contract with Liaison Care provided the ICB with a transparent and value-driven arrangement, ensuring that resources were allocated efficiently.
Liaison Care’s commitment to a person-centred approach ensured that the standard of care remained excellent, meeting the individual needs of patients and their loved ones.
The cost per case model not only provided reassurance to the ICB about the effectiveness of the partnership but also demonstrated tangible value for each case completed, contributing to the overall improvement of healthcare services in the region.
